From the 5 Clarence Faflik Shoe Stores in Cleveland Ohio in the 1940's - 1950's.  An 8 panel fold out with spaces for stamps on 6 of the panels. 5 panels are full of stamps and 1 panel has 1 stamp.  and another panel which was used for the customer information has 20 stamps. If half of the folder has been complete the customer gets .00 in merchandise and for filling the entire folder the customer gets .00 in merchandise. Faflik carried John C Roberts, Air-Step,Red Goose, Jumping Jack and Cosyfoot Shoes. Faflik Shoes went out of business in the 1950's.
